Background

Smoky Lake Public Library purchased a DAISY Reader (VictorReader ® Classic X+) with funds from a CAP (Community Access Program) grant in 2006.

Policy

1. Only local cardholders in good standing of the Smoky Lake Public Library may borrow the DAISY Reader.

2. The borrower must be verified by the Library Manager as “visually impaired, or unable to read regular print materials due to illness or learning, physical and/or orthopedic disability” as outlined in The Alberta Library (TAL) agreement with the Canadian National Institute for the Blind (CNIB).

3. The borrower must be eighteen (18) years of age or older, or have a parent or guardian sign out the VictorReader ® on his/her behalf.

4. The loan period shall be three (3) weeks with renewals permitted.

5. Damage or loss of reader on loan is the responsibility of the cardholder.

6. Cardholder shall be charged for loss, theft or damage of the Reader.
