With Cat Sheet Direct, you can keep your items on hand while still getting them professionally catalogued. It’s quick, convenient, and reduces the time you spend preparing items for van run and waiting for shipments to and from NLLS.
This form is intended for any managers or staff who are authorized to have items catalogued. You'll find the form at the end of this page.
You’ll only need to provide a few details—most of which you already include on traditional cat sheets. When filling out the form, please have the following information ready:
- Title
- Author
- ISBN
- Barcode
- Number of Pages
- Collection and Template (according to your cataloging profile)
- Cost
- Notes (optional)
- Confirmation that you’ve checked Polaris to ensure the title isn’t already in the system
You’ll notice a few new fields—these help our catalogers maintain the high standards you’re used to.
Once submitted, the form goes straight to our staff for cataloging
The form can be accessed below — it won’t be linked elsewhere, so please bookmark it or save the link for future use. The weekly 24 item limit is still in effect.
If you come across a title that matches an existing one in Polaris, with the same author but has a different ISBN, we still ask that you use this form so we can add the ISBN to the existing record.
